<div> <ul> <li data-start="2078" data-end="2211">Evaluate patients using appropriate tests and measurements to determine level of function and create individualized treatment plans</li> <li data-start="2214" data-end="2300">Establish physical therapy interventions based on physician orders and patient goals</li> <li data-start="2303" data-end="2384">Deliver competent and safe therapy techniques in accordance with best practices</li> <li data-start="2387" data-end="2467">Ensure continuity of care through effective communication with other providers</li> <li data-start="2470" data-end="2535">Document patient progress and submit billing in a timely manner</li> <li data-start="2538" data-end="2614">Provide patient/family education on home programs and self-care as a Physical Therapist in the State of Colorado, or hold a valid PT Compact Privilege</li> <li data-start="2963" data-end="3022">Hold a current Basic Life Support (BLS) certification</li> <li data-start="3025" data-end="3072">Strong interpersonal and communication skills</li> <li data-start="3075" data-end="3133">Committed to evidence-based practice and clinical growth</li> <li
